Insurance coverage
For Las Vegas residents, a prescription and an insurance approval are separate decisions. This guide explains formularies, prior authorization and follow-up requirements so you can ask about the benefit attached to your own plan.
What plans ask
Begin with the exact product and the reason it is being prescribed. A formulary is the plan's list of covered drugs, but a listed medicine can still have restrictions. Ask whether the benefit covers that product for the documented condition, which pharmacy must dispense it and whether authorization is needed. The prescription medication overview helps distinguish products that can sound similar in a coverage conversation.
Prior authorization is a request for the payer to review information before it agrees to cover treatment. The prescriber may need to provide clinical records or explain previous treatment. Our weight loss eligibility guide concerns medical assessment; it does not establish an insurer's criteria. Ask the plan for its current policy and the provider who will submit the requested information.
Coverage can also depend on quantity limits or requirements involving previous treatments. Record what the plan actually requests rather than relying on another patient's approval. The weight loss coverage check helps organize a plan-specific inquiry. A provider saying it accepts your insurance establishes neither a medication benefit nor the amount you would pay for a particular prescription.
The process
Is GLP-1 for weight management a covered benefit, is prior authorisation required, and what documentation is needed. Get the answers in writing if you can.
Recorded BMI, the qualifying diagnosis, and any documented previous attempts. Gaps here are the most common reason for refusal.
A prior authorisation supported by proper clinical notes is assessed very differently from a bare request.
Prior authorisation is rarely same-day. Build that into when you plan to start rather than being surprised by it.
If refused
When a decision arrives, check the named medicine, authorized period and any refill or renewal conditions. Ask the pharmacy for your expected payment and confirm whether a deductible or coinsurance applies. An approval letter may not cover every related appointment or test. Our weight loss program costs page explains why care fees need a separate check, while the medication price guide addresses pharmacy quotes. Keep the decision and the policy it cites so you can identify a change later.
If coverage is denied, read the stated reason and deadline before choosing a next step. Missing records, a formulary restriction and an excluded benefit can require different responses. Ask the plan about its exception or appeal process and the prescriber about appropriate alternatives; a request does not guarantee approval. Medicare also has specific pathways, including the temporary GLP-1 Bridge for eligible Part D beneficiaries and selected products from July 2026. Verify current rules with the payer. The weight loss cost overview can help you account for expenses while a decision is pending.

Common questions
The written policy and decision for your own plan are the useful starting points.
Given a list of exactly what my insurer would want. Approved on the first submission, which I am told is unusual.
First claim failed on a missing diagnosis date. They helped fix it and the second went through.
My plan excluded it outright. They said so straight away rather than letting me waste weeks appealing.
Started the authorisation before my consultation so the approval landed around the same time. Saved a month.

Next step
Ask your insurer three questions before you start: is it covered, is prior authorisation needed, and what documentation do they want.
